The Challenge
The majority of the general population now suffers from one or more Chronic Conditions and as the Baby Boom generation ages, the problem is worsening.  Disease Management companies and Specialists handle a portion of these patients, however, PCPs are still the frontline when it comes to Chronic Condition Management.

How we can help
In contrast to Integrated Behavioral Health programs where the issue is around detection of under-diagnosed conditions, Chronic Conditions generally are already known, but need to be managed.  Mirroring your practice's treatment protocols: registries are setup (diabetes, asthma, hypertension, chronic pain, etc), data collection protocols are configured and questions & measures are assigned (compliance, severity, co-morbid psych, quality-of-life, etc).

With these data protocol definitions setup, based on the patient's known Chronic Conditions, trigger logic in the Electronic Patient Interview automatically collects the required patient information during each interview.  Integrating this data into the patient's treatment, provides the PCP with key information, as well as information they normally would not have the time to gather, making them more effective in the management of the patient's Chronic Condition.

Looking at Outcomes
Using Patient Tools to manage the data collection process you can instantly view how well your patient population is doing.  You can see how well Chronic Condition treatment protocols are working on average, compared to norms or set points.  You can also drill down to see how well sub-groups are fairing or look at individual cases.
