Overview
Primary Care is an ideal application for Patient Tools' labor saving automation.
Using trigger logic to combine screening, simple intake and
in-depth assessment in a Pre-Visit Electronic Patient Interview, a
PCP can gather and respond to a range of patient information that
they ordinarily do not have the time to address. This
proactive, more efficient use of the PCP's time, leads to
healthier patients, reduced costs and better access.
Pre-screens & In-depth Assessment when appropriate
One of the advantages of trigger logic is the ability to
electronically ask pre-screen questions and based on the response,
trigger a more in-depth set of questions as indicated.
Alternately the PCP or MA can ask the pre-screen questions and
then decide to administer an in-depth assessment. While
either method works, PCPs are finding trigger logic produces more
consistent results and further reduces the workload on the PCP and
their staff.
Annual Protocol
Combine pre-screening & trigger logic with an annual protocol
(initial visit, annual sample points) and you have the tools to
screen patients into registries/programs, as well as measure the
performance of these programs. An annual protocol is easy
for staff to administer and it gives the practice an in-depth
overview of its patient population. Aggregate initial visit
information characterizes the patient population, while aggregate
annual visit information measures progress towards program goals.

Clinical View -
Interview
patients electronically before and/or after any patient visit,
enabling trended individual patient data;
computed, summarized and reported to the provider before they
start the visit. You utilize your patients as a resource
for data entry and the system reports the results with

Program View - Population Management using aggregate patient data, organized
into registries and protocols. Integrated Behavioral
Health, Chronic Condition Management and
Wellness Initiatives
are excellent programs that your practice can initiate with
little overhead since they can be seamlessly integrated into the
Pre-Visit Electronic Patient Interview.
System View - Quality Management &
Satisfaction using aggregate patient feedback data and
performance metrics can be included in a Post-Visit Electronic
Patient Interview.
